Individual Tax Return Questionnaire

These forms are .PDF files.  The Personal Data form is 'fill in' then print.  All other forms are print then manually fill in.

 

Personal Data

Personal data required to complete your return.

Income

Did you have other income not usually reported on a W-2, 1099, etc.  Examples include alimony, scholarships, tip income, jury duty, rental of personal property, seller-financed mortgage interest, interest received on personal loans, etc. 

Have foreign income or pay any foreign taxes

Receive stock from a stock bonus plan

Exercise stock options

Child and/or Dependent Care Expenses

Complete this form if you had any child or dependent care expenses

Make or plan to make IRA contributions

The "assets" form can be used to report the basis or purchase price of stocks and securities.  The sales price is reported on Form 1099-B, etc.

The asset form is often required with these activities.  It is used to report the basis of rental property, the acquisition of appliances, etc.
